Forms
Every Grant Application, Grant Interim and Grant Final Report requires a Project Budget Form.
You can download a Project Budget form here. (There is some extraneous language at the top of the form for folks who are able to download the form from Google Sheets. If you are downloading from here, you can disregard that.)
Award packets announcing your grant will give you an approximate due date for your report, but we will also notify you via email that a report is due soon, and provide directions to access and complete the report in the UUFP Funding Center. It should be short and easy, pre-populating some of the information for you directly from your original Application!
An Interim Report should be included with a new proposal if the work on the previous year’s grant project is incomplete or not spent down at the time of application, approximately four months prior to the end of the grant year.
This is an example of a Grant Interim Report.
Final reports are the main tool we use to gauge our success as a funder, and are a key part of our decision making process if you seek future grants. If you received funding from us in the past 5 years, a Final Report should be attached. If you filed a Final Report on the UUFP Funding Center, you can download a .pdf of that report. If you need assistance locating and printing that, please email us, and we'll be happy to assist you.

If you need to submit a Report for Grants made the Fall 2019 cycle (awarded in January 2020) or earlier, please email us to obtain a .pdf of the form.
With the exception of certain special grants made by the Fund for UU Social Responsibility, most grants are for a period of one year. The Final Report is expected at 13 months (July 31 for grants made in the Spring cycle of the previous year, and January 31 for grants made in the Fall cycle of the year 2 calendar years earlier).
